Abstract

THE ICD has made a dramatic impact on the management of patients with ventricular tachycardia (VT) and fibrillation (VF), markedly reducing the risk of sudden cardiac death in this high-risk group. In addition, the ICD has evolved from a relatively simple, non-programmable device to a complex unit with multiple options for tachycardia detection and therapy. As ICD technology has developed, a variety of clinical scenarios have arisen which force the electrophysiologist to differentiate appropriate from inappropriate device function. This chapter seeks to outline essentials of routine ICD clinical followup and then identify the most common types of clinical problems, other than infection, encountered during ICD followup. (Table 1) The differential diagnosis and plan for management of these problems will be discussed.
